When traveling, I have found that being able to find a bathroom is crucial on occation. Having access to a nice bathroom is even better.
In Europe, a bathroom is just that in some places - a room you take a bath in. Look for Water Closets, or just WC. Toilet is not always a good indication, either, though in England and France, you have a good likelyhood of finding a proper facillity. And, bring your own paper when not in the US.
In the United States, most restaurants have to have a bathroom connected to be able to serve food. New York City seems to be exempt from that one, though. If you do use a restaurants' bathroom, please buy something!
In Madison, Wisconsin, I have come accrost many good, and bad, bathrooms.

I reccomend finding the Madison Club in the downtown area for a good set of bathrooms. They have clean rooms, and good extras, like lotions and hairspray. They are butted up against the Monona Terrace.
The Inferno night club off of Sherman ave. on the east side has a great sence of style in the industrial steel and paint. The night club it's self is loud, smoky and has bad prices, but if you are looking for a bathroom with style, this is one!
305 - 307 Patterson Street. This is on the near east side of Madison, off of Williamson Street. They have nice bathrooms with extras, and the men's as well as the women's bathroom is kept clean.
Benvenuto's has a good, open plan for the bathroom - excelent for wheelchairs... until you get into the stalls. They only have one wheelchair accessable stall per gender, but the cleanliness and light are great.
A bathroom to avoid is in the Edible Complex off of Applegate Road and Greenway Cross. This south side resturant/bar/nightclub is a wonderful place to have a drink, prime rib on Saturdays and dance with your hunny, but catheterize yourself so that you do not need to 'go' while there.
